Adopting a dog is a lifelong commitment, and we want to make sure that there’s a good match between you and the selected dog. There are no right or wrong answers – your honest responses will help us guide you to the best dog for your unique situation.

Consent to Background Check and Liability Waiver

As part of our commitment to ensuring the safety and well-being of the animals in our care, Dog World Rescue requires all applicants to undergo a background check as part of the adoption/foster application process. By submitting this application, you hereby consent to a background check conducted by Dog World Rescue or its designated third-party provider. This background check may include, but is not limited to, a review of criminal history, previous involvement in animal-related incidents, and other relevant factors to determine your eligibility.

Confidentiality and Privacy

Your personal information will be handled with the utmost confidentiality, and results of the background check will only be used for the purpose of evaluating your application. We comply with all applicable privacy laws and will not share your information without your consent, except as required by law.

Non-Discrimination

Dog World Rescue is committed to equal opportunity and will not discriminate based on race, gender, religion, national origin, or any other protected class in conducting the background check. However, we reserve the right to decline any application based on the results of the background check if it is deemed not in the best interest of the animal.

Disclaimer and Liability Waiver

By submitting this application, you acknowledge and agree that Dog World Rescue is not liable for any direct, indirect, or consequential damages arising from the background check process. You also release Dog World Rescue from any liability should your application be denied as a result of the background check findings.

I understand that if I do adopt a DWR dog, I will sign a contract and this application will become part of that contract. Home visits and follow up calls may be made on a random basis prior to or following adoption. If any information contained in this application is found to be false, I understand that the adoption contract will be considered null and void, and the dog maybe reclaimed by DWR, without a refund of monies paid.
